How To Obtain Disability Placard
Disability Placard applications are available by clicking here. Once completed and signed by a medical doctor, return the application to Police Headquarters along with a $4.00 check made payable to New Jersey Motor Vehicle Commission.
If an extension is needed, please come to Police Headquarters on or after the expired date (unless it falls on a weekend, but not before) with an application for renewal along with a $4.00 check made payable to NJMVC.
